During this period, your usual GP Practice service has been diverted to 2 physical GP hub locations across the borough and a home visiting service.
If you require urgent, same day, medical advice or assistance please call 0208 623 1304. You will be diverted to a care navigator to assist you further and aim to provide you with an appointment where appropriate which may include a face-to-face hub appointment, a telephone call back from a clinician and/or a home visit.
In case of an emergency or if you need urgent medical attention outside of our practice hours, please dial NHS 111 or 999 for immediate assistance.
