Do I need to re-register?
If you were previously a patient at either Church Lane or Cannon hill Lane Medical Practices you have been automatically registered with The Nelson Medical Practice. You do not need to complete a new patient registration form. If you have any questions please contact one of our Patient Care Navigators.
Who can register?
Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.
As a large and visible practice in the heart of the borough we know that many refugees arriving in Merton may wish to register with us. We want to ensure that registering with a GP is as easy as possible and the care received is of the highest quality. If you’re unsure, please ask reception for help with your registration form, all our team are happy to help.
We are able to register patients from anywhere in London and the surrounding areas. Unfortunately, we are only able to offer home visits to patients who live within one of our practice boundaries. Please note patients who are out of area but still wish to register will need to come into the practice and sign a form showing their acceptance of this condition.
