The Nelson Medical Practice was formed in 2015 with the merger of The Church Lane and Cannon Hill Lane Medical Practices.

The Nelson Health Centre, is built on the former site of the Nelson Hospital and is managed by Community Health Partnerships (CHP). The Centre aims to provide the best quality care for the local community.

Please note: only patients who live within our practice boundary will be able to have home visits.
